From Golden Hills Software:SurveyGold is a program designed for building, administering, and analyzing the results of surveys. It is intended for individuals and organizations that need to quickly create and conduct surveys over the Web, the phone, or using printed forms. It provides point-and-click features for collecting and reviewing survey results. You can create Web-based survey forms that email responses directly to you. The program also integrates with Microsoft Word and Outlook. SurveyGold 6.0 provides new support for conducting and scoring exams. It also provides numerous enhancements to Web surveys including required fields.
Note: In the unregistered version, you are limited to maintaining three surveys, round-trip Web surveys are not enabled, and the Word-Reporting feature is not enabled. The registration fee is $99.95.
